Lifeworks in Norwood, MA is seeking a full-time Case Manager to support adults with developmental and intellectual disabilities. You will help individuals pursue employment aligned with goals, coordinate supports, and maintain records in a collaborative team environment.

Responsibilities include writing ISP plans, tracking progress, and delivering Life Enhancement Curriculum activities, with a focus on empowerment and community participation. Competitive benefits are available.

Qualifications

• Experience serving individuals with ID/DD in a Human Services setting.
• Bachelor's or Associate degree in a related field; or high school diploma with related experience.
• Must be able to pass background checks and have a valid driver's license.
• Strong verbal and written communication skills and documentation ability.

Responsabilités

• Assist individuals served within the program to obtain and maintain employment aligned with goals.
• Collaborate with a team to plan advocacy and support, attending regular meetings.
• Support self-employment, education, social growth, and health wellness activities.
• Provide case management support and maintain electronic and physical records.
• Write and implement ISP plans and complete documentation; maintain progress notes.
• Engage in on/off-site Life Enhancement Curriculum activities and payroll/billing tasks.
• Perform duties across locations as needed to support the team and individuals.
