Responsibilities

• Plan, develop, organize, implement, evaluate, and direct the activity programs of the facility.
• Interact with residents, staff, visitors, and external agencies under all conditions.
• Review and participate in policy/procedure updates related to activities.
• Coordinate with other departments and participate in discharge planning and resident assessments.
• Assist with arranging transportation and provide information on Medicare/Medicaid eligibility and other assistance as needed.
• Maintain relationships with medical staff and other health facilities; report to Administrator on activity department operations.

We are looking for Activities Director for the team.

Duties

• Plan, Develop, Organize, Implement, Evaluate. And direct the activity programs of this facility.
• Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
• Review department policies and procedures, at least manually, and participate in making recommended changes
• Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
• Participate in discharge planning, development and implementation of activity care plans and resident assessments.
• Interview resident/families as necessary and in private setting.
• Assist in arranging transportation to other facilities when necessary.
• Provide information to resident/families as to Medicare/Medicaid eligibility, and other financial assistance programs available to the resident, as necessary.
• Assume the authority, responsibility, and accountability of directing the activity department.
• Maintain a productive working relationship with the medical profession and other health related facilities and organizations.
• Coordinate activities with other departments as necessary.
• Make written and oral reports/recommendations to the Administrator concerning the operation of the activity department
• Be able to assist and coordinate family visits.
