The Waters of Sullivan in Sullivan, IN is seeking an Activity Director to lead the resident activity program in a skilled nursing facility. You will plan and direct activities, coordinate with staff, and ensure residents' physical, mental and psychosocial well-being while complying with regulations.

Qualifications

• Bachelor's Degree (preferred).
• 3 years experience in Activities Department in a Skilled Nursing Facility, 2 of which must have been as an Activity Director.

Responsibilities

• Plan, organize and direct the Activity Department to meet residents' physical, mental and psychosocial well-being.
• Facilitate group activities.
• Encourage activity involvement and chart attendance.
• Assist with activity supplies, cleanliness and organization.
• Assist with 1-on-1 activities and transport residents.
• Assist with outings and special events.
• Collaborate with all disciplines for patient-centered care.
• Ensure confidentiality, HIPPA and Residents' Rights compliance.

Duties and Responsibilities:

• Planning, organizing, developing and directing the overall operation of the Activity Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator, to assure that an on-going program of activities is designed to meet, in accordance with the comprehensive assessment, the interests and the physical, mental and psychosocial well-being of each resident.

• Facilitate group activities

• Encourage activity involvement and chart attendance

• Assist in keeping activity supplies clean and organized

• Assist with 1 on 1 activities, transport residents to and from activities

• Assist with outings and special events and other assigned duties

• Collaborates with all disciplines to provide best patient centered treatment and care.

• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications/Requirements:

• Bachelors Degree (preferred)

• 3 years experience in Activities Department in a Skilled Nursing Facility, 2 of which must have been as an Activity Director
