Job summary

Live Long Well Care of Tega Cay, SC, seeks an Operational Support Liaison to represent the agency as a front-line customer service liaison between patients, clients, caregivers, the CSM and CCS.

Under the supervision of the CSM and Care Coordinator Supervisor, you will assign the same caregiver to every patient/client when possible, maintain weekly schedules, verify documentation, and support the continuity of care and patient satisfaction.

Qualifications

• Knowledgeable of and supports the agency's philosophy, purpose, mission, goals and policies.
• Supports the needs of the patient/client.
• Maintains a standardized weekly schedule for all active patients/clients and caregiver assignments.

Responsibilities

• Represent the agency as a front-line customer service liaison between patient/clients, caregivers, the CSM and the CCS.
• Identify and assign the same caregiver to each patient/client whenever possible to ensure continuity of care.
• Maintain weekly schedules and document daily scheduling changes.
• Validate caregiver documentation against schedules prior to payroll.
• Communicate with the Care Coordinator Supervisor to ensure staffing meets policy requirements.
