Job summary

Qualifications

• 1+ years in intake, screening, front-desk, or client-services work.
• Experience entering client data in CaseWorthy or HMIS.
• Ability to recognize urgent or crisis situations and follow escalation steps.
• Basic Excel or Google Sheets.

Responsibilities

• Receive walk-ins, calls, and referrals as the first contact at the front door.
• Run the standard coordinated entry assessment at intake.
• Prioritize each person by need and urgency.
• Follow escalation steps when a person is in crisis.
• Enter every contact in CaseWorthy accurately and on time.
• Capture eligibility documents at intake and keep records audit-ready.
• Route clients to the right program with warm handoffs.
• Follow up on pending handoffs to prevent gaps.
