Job summary

Qualifications

• At least 2 years experience in health care, care management, or related settings.
• Clinical licensure and/or certification required only if state contract requires it.
• Valid driver's license and reliable transportation for job-related travel.
• Knowledge of community resources and ability to engage diverse populations.
• Ability to work independently with minimal supervision.

Responsibilities

• Completes member assessments and determines care coordination eligibility.
• Develops care plans with member, caregiver, and health professionals.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness and documents progress.
• Maintains ongoing member caseload and coordinates services across teams.
• Promotes integration of behavioral health, LTSS, and community resources.
• Facilitates ICT meetings and collaborates with care teams.
• Uses motivational interviewing to support change.
• Identifies barriers and assists members to address concerns.
• Engages with licensed care managers as needed.
• 25–40% local travel may be required.
