Qualifications

• At least 2 years of experience in health care, care management, or related settings.
• Clinical licensure/certification required only if state contract mandates.
• Valid driver's license and reliable transportation for travel.
• Ability to work across diverse populations and settings.

Responsibilities

• Completes assessments of members per timelines and identifies who needs care coordination.
• Develops care plans with member, caregiver and health professionals to meet goals.
• Performs telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness and documents interventions and outcomes.
• Maintains ongoing member caseload and outreach.
• Promotes integration of behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary team meetings and collaboration.
• Uses motivational interviewing to support and motivate change.
• Assesses barriers to care and provides necessary assistance.
• Collaborates with licensed care managers and leadership as needed.
• Travel 25-40% based on state/contract requirements.
