Qualifications

• 2 years of healthcare experience in care management or equivalent
• Valid driver's license, reliable transportation, and auto insurance for travel
• Knowledge of community resources
• Ability to work proactively, with attention to detail
• Ability to work in diverse settings and with diverse populations
• Strong written and verbal communication skills
• Proficiency with Microsoft Office and related software

Responsabilités

• Completes member assessments per timelines to determine care coordination eligibility
• Develops and implements care plans with member, caregiver, and providers
• Conducts telephonic, in-person, or home visits as required
• Monitors care plan effectiveness, documents interventions and outcomes
• Maintains ongoing member caseload and conducts regular outreach
• Promotes integration of behavioral health, LTSS, and community resources
• Facilitates interdisciplinary care team meetings
• Uses motivational interviewing to educate and support change
• Assesses barriers to care and coordinates with leadership as needed
• Travel 25-40% may be required based on state/contractual requirements
