Job summary

National Youth Advocate Program in Columbus, Ohio seeks an entry-level Treatment Coordinator to join the Foster Care Team as a full-time position. You will work in the community, providing advocacy and support toward safe, successful community living under supervision.

The role involves coordinating mental health services, daily travel to youth and families, and collaboration with foster parents and community partners.

Qualifications

• A Bachelor's Degree in Social Work from an accredited institution.
• Valid and active LSW license, preferred. Willingness to work flexible and non-traditional hours including evenings, at least two per week.
• Proficient use of desktop and laptop computers, smartphones and tablets, printers, fax machines and software (word processing, spreadsheets, databases).

Responsibilities

• Provides individual, group, and family mental health services and other evidence-based clinical services as required.
• Travels daily to provide community-based services to youth and families in compliance with organizational, contract, and regulatory requirements.
• Coordinates and monitors clinical services for youth and family in the community, including mental health, education services, medication management and psychological services.
• Regularly reviews and modifies youth-based plans.
• Establishes and maintains strong relationships with foster parents and families of origin.
• Assists in family reunification and stabilization efforts in line with the youth's case plan.
• Provides safety assessment, crisis response, and behavioral stabilization services as required.
