Job summary

Family Care for Children & Youth, Inc. seeks a Foster Care Permanency Case Manager to support children in care and their resource families. You will monitor progress, prepare service plans, and collaborate with the team to ensure safety and stability.

The role involves attending court hearings, testifying when required, and recruiting families while maintaining meticulous files and coordinating services for children and families.

Qualifications

• Bachelor’s degree in social work, social sciences, or related field.
• Valid Pennsylvania driver’s license.
• Childline, FBI, PA State Police, and National Sex Offender Clearances required.
• Evening hours and on-call weekends availability.
• Excellent verbal and written communication skills.

Responsibilities

• Meet with children in care and resource families to monitor progress.
• Complete Contact Logs, Progress Reports, and Individual Service Plans.
• Attend court hearings and testify as required.
• Recruit resource families and conduct family studies and reevaluations.
• Maintain client and family files and coordinate services.
