Job summary

The position is full-time in a not-for-profit setting, with a required degree in social work or related field and at least two years in relevant services. Evening/weekend hours and CPR/First Aid are noted as needed.

Qualifications

• Bachelor’s Degree (Master’s preferred) in Social Work, Human Services, or a related field.
• At least two years of experience in foster care, social services, or case management.
• Strong problem-solving, decision-making, and organizational skills.
• Advanced writing skills for documentation, reports, and communication.

Responsibilities

• Accept and manage all referrals for placement, ensuring completion of intake documentation.
• Participate in orientation, training, evaluation, and assessment of prospective foster family home applicants.
• Establish and maintain a referral matching system, tracking discharges within 30, 60, 90 days.
• Ensure all documentation is uploaded in Child Trax and meets standards.
• Evaluate and assess children for placement based on needs and permanency plan.
• Conduct home visits and foster parent support meetings, including evenings/weekends.
• Provide pre-service and in-service training for foster parents, incl. weekend sessions.
• Participate in on-call rotation for emergency and crisis support.
• Prepare court reports and attend judicial placement reviews.
