HealthCare Access Maryland (HCAM) is seeking a Mental Health Case Manager to coordinate medical and behavioral health services for children in the Baltimore City foster care system. The role involves collaboration with BCDSS, caregivers, and providers to ensure timely access to care and resources, with strong organizational and communication skills.

Applicants should have a Master’s in Social Work or related field and at least one year of relevant experience working with children and families.

Qualifications

• Master’s degree in Social Work or related Human Services field.
• Minimum one year of experience working with children and youth.
• Experience serving families and children in public health or community settings.

Responsibilities

• Coordinate healthcare for children in foster care with a multidisciplinary team.
• Ensure timely medical and mental health care access; support caregivers and child welfare staff.
• Document care plans, assessments, medications and records in multiple databases.
• Participate in meetings and coordinate case assignments and services, prioritizing urgent needs.
• Navigate HealthChoice system and coordinate with partners to resolve barriers.
