Job summary

Star of Hope Family Services in Fairmont, WV is seeking a Foster Care Home Evaluator & Treatment Coordinator to recruit, license, train, and support foster and adoptive families while providing case management and treatment coordination for children in care.

The role ensures compliance with agency, state, and federal regulations, develops treatment plans every 90 days, coordinates services, and participates in MDT meetings when required.

Qualifications

• Bachelor's degree in Social Work or related field from an accredited college or university.
• Social Work licensure preferred and may be required in accordance with West Virginia regulations.
• Minimum of three years of postgraduate experience in social work, child welfare, foster care, or a related field of human services.
• Knowledge of child welfare principles, licensing standards, and applicable federal and state regulations.
• Strong assessment, interviewing, case management, and treatment planning skills.

Responsibilities

• Recruit foster/adoptive families through community outreach.
• Open new foster homes and conduct home studies.
• Develop and implement licensing and safety reviews.
• Coordinate medical, behavioral health, educational services for children.
• Maintain licensing and documentation in agency systems.
• Participate in MDT meetings and court proceedings when required.
