Job summary

Children's Aid is seeking a Case Planner in New York, NY. You will collaborate with a multidisciplinary team to manage a caseload of about 10 children with diverse mental health needs, delivering intensive casework and field services to promote permanency.

Qualifications include a Bachelor's Degree and driver’s license; Foster Care experience and bilingual Spanish/English are preferred. The role supports a hybrid in-person/remote schedule within a mission-driven organization.

Responsibilities

• Utilize the CONNECTIONS system and maintain written case records.
• Coordinate Family Team Conferences and participate in case consultations.
• Work in a hybrid schedule to support team collaboration.
