Job summary

The New York Foundling is seeking a Foster Care Case Planner in New York, NY to support permanency goals for children through reunification, adoption, or independent living planning. The role emphasizes trauma-informed care, service coordination, and advocacy with families and service providers.

Qualifications

• BA/MA in Psychology, Sociology, BSW or MSW with 1-2 years child welfare experience preferred.
• Knowledge of community resources and ability to negotiate systems for multi-problem families.
• Experience with drug treatment populations preferred.
• Typing skills and computer literacy required.
• Excellent interpersonal skills.

Responsibilities

• Maintain a caseload of 12 cases on average.
• Attend all court proceedings on time with prepared court reports and/or permanency reports.
• Conduct home visits to the homes of foster parents and birth parents / discharge resources.
• Coordinate and facilitate family and sibling visitations/meetings; supervise as required.
• Provide extensive outreach/advocacy services; connect families to community resources.
• Any other duties as assigned.
