Job summary

Good Shepherd Services in New York is seeking a dynamic Case Planner to advance permanency planning for children and youth in foster care. You will assess safety, coordinate visits, and work with families and schools to support outcomes.

The role requires a related bachelor’s degree (MSW preferred), strong communication, and ability to partner with KEEP/PTC teams. Hybrid in-office/field schedule and 35 hours per week.

Qualifications

• Bachelor’s degree in a related field is required.
• MSW or related experience preferred.
• Ability to engage clients and families.
• Excellent written and verbal communication.
• Willingness to facilitate groups with parents and foster parents.
• Ability to work with KEEP/PTC/YDSC models and practice.

Responsibilities

• Assess and monitor for safety and risk for children in care.
• Establish contact with families and foster families to coordinate care.
• Conduct required casework contacts with child, parent, and service providers.
• Visit family and child to gather social history and assess needs.
• Coordinate and supervise visits between family and child and siblings.
• Maintain up-to-date case records and complete reports per agency standards.
• Attend hearings and participate in Family Team Conferences as required.
