Job summary

The New York Foundling seeks a Foster Care Case Planner to advance permanency for children through reunification, adoption, or independent living planning. You will work with birth families, foster families, and youth using evidence-based practices and participate in multidisciplinary teams to deliver strong, coordinated care.

Responsibilities include documenting accurately, coordinating referrals, attending court and immigration meetings, and managing a caseload of about 12.

Qualifications

• Bachelor's Degree in Psychology or Social Work
• 2+ years of Child Welfare experience
• Experience working with adolescents
• Excellent communication skills
• Knowledge in adolescent development and pregnancy
• Effective organization and time management
• Good computer skills; able to use Connections
• Strong interpersonal and team-oriented skills

Responsibilities

• Engage with birth parents, foster parents and youth using motivational interviewing and assess safety and permanency factors.
• Ensure timely documentation in accordance with regulations for service planning.
• Participate in coaching to promote professional development and teamwork.
• Attend interdisciplinary team meetings to foster collaboration for permanency.
• Complete referrals for services to ensure continuity of care.
• Attend court hearings and immigration appointments to support permanency outcomes.
• Ensure service plan goals are implemented to support youth and casework.
• Maintain a caseload of 12 cases on average.
