Job summary

Necco seeks a Foster Care Case Manager to collaborate with children and foster families across office, community, school, and home settings. The role focuses on creating and monitoring individualized treatment plans, coordinating services, and ensuring safety and regulatory compliance.

The position requires 21+ years of age, licensure or bachelor’s degree in a human services field, and willingness to travel.

Qualifications

• Minimum age 21 years or older.
• Must have a valid driver’s license.
• Criminal background checks and drug screenings completed as required.
• Requires Bachelor’s or higher in a human services field and relevant licensure.
• Willingness to travel for training and field work.

Responsibilities

• Participate in referral matching for children and families, advocating for best homematch.
• Develop Initial Treatment Plan and Comprehensive Treatment Plan.
• Complete biopsychosocial, Independent Living, and Diagnostic assessments.
• Refer foster child to behavioral health, independent living, and other services.
• Ensure initial legal documentation and safety plans are in place.
• Develop and facilitate Individualized Treatment Plans (ITP).
• Support implementat ion and monitoring of treatment plans with foster parents and children.
• Meet and exceed state/licensure regulations regarding contact with caseload.
• Arrange and document medical and therapy services per service plan.
• Serve as liaison among Necco, placement agency, and community resources.
• Participate in service meetings with multiple stakeholders.
• Establish relationships with school personnel to meet education goals.
• Provide ongoing training and support to foster parents and children.
• Monitor documentation for state/licensure and accreditation compliance.
• Maintain confidentiality as required by policy.
• Assist CAP tasks for foster parents and monitor medication logs.
