Job description

Pinnacle Family Services provides evidence-based therapeutic foster care and behavioral health services to at-risk youth in North Carolina and South Carolina. We specialize in children who have experienced trauma, loss, abuse, or neglect.

Foster Care Case Manager

As a Foster Care Case Manager, you will oversee a caseload of children and families, ensuring coordinated, compassionate care, individualized treatment plans, and consistent support.

What You’ll Do
  • Develop and implement individualized treatment and service plans that support each child’s safety, stability, and well‑being.
  • Coordinate medical, educational, behavioral health, and social services to ensure seamless, uninterrupted care.
  • Serve as a central point of communication among foster parents, biological families, service providers, and internal teams.
  • Facilitate and support healthy interactions between children, foster families, and birth families to promote positive outcomes.
  • Monitor and continuously assess each child’s progress, adjusting care plans to meet evolving needs.
  • Maintain accurate, timely documentation and case records in accordance with organizational and regulatory standards.
  • Partner with foster parents to provide guidance, support, and resources that help create safe, nurturing home environments.
  • Collaborate with internal teams and external agencies to ensure coordinated service delivery and compliance with all requirements.

We require strong organization, adaptability, and the ability to manage multiple priorities in a dynamic environment. You’ll work closely with various stakeholders, balance urgent and long‑term needs, and communicate clearly, problem‑solve proactively, and maintain attention to detail.

Supporting Quality & Compliance

All services and documentation must meet Health Connect America, state, and Council on Accreditation standards. You will maintain high‑quality work, meet required timelines, and contribute to continuous improvement efforts.

Qualifications
  • Bachelor’s degree in Human Services with a minimum of two years of experience working with youth or families, or in a non‑Human Services field with a minimum of four years of experience working with youth or families.
  • Experience serving children and adolescents with a primary mental health diagnosis, meeting Qualified Professional qualification.
