Job summary

Necco is seeking a Foster Care Case Manager to support children and foster families across office, school, and community settings. You will help children reach their goals through coordinated treatment plans and community resources.

You will collaborate with foster parents, schools, and agencies to ensure safety, compliance, and permanency. Travel to foster homes and training will be required as essential duties.

Qualifications

  • 21 years of age.
  • Current LSW or LPC or Bachelor's degree in a human services‑related field (OH, KY, GA).
  • Bachelor's degree with field experience of 2 years (WV).
  • Bachelor's degree or Master's degree from an accredited four‑year college or university (Tennessee).
  • The degree must be supplemented by at least 7 hours in behavioral sciences, such as social work, psychology, and sociology.
  • One year of casework experience can be substituted for the 7 hours.
  • Minimum of 2 years‑experience working with children with emotional or behavioral problems.
  • Valid Driver's License.
  • Must have the skills necessary to navigate and execute several software’s (i.e. Microsoft offices, Databases, Electronic Health Records, Dashboards, etc.).
  • Successful completion of all required Criminal Background checks and drug screenings per state and company regulations.
  • Ability to adhere to scheduled and unplanned deadlines.
  • Willingness to travel and attend training.

Responsibilities

  • Participate in referral matching and advocate for best home match.
  • Develop and monitor Initial Treatment Plans and Comprehensive Treatment Plans.
  • Facilitate assessments such as Biopsychosocial, Independent Living, and Diagnostic assessments.
  • Refer foster child to applicable support services (behavioral health, independent living, etc.).
  • Ensure initial legal documentation is obtained prior to placement or service delivery.
  • Ensure safety plans are developed during intake.
  • Develop and facilitate each consumer's Individualized Treatment Plan (ITP).
  • Assist with implementation, tracking, and monitoring of the Comprehensive Treatment Plan.
  • Establish relationships with foster parents and children through training and support.
  • Meet/state licensure regulations regarding contact with caseload.
  • Ensure foster children receive medical and therapy services per service plan.
  • Serve as liaison between Necco and placement agencies and community resources.
  • Participate in service meetings with stakeholders.
  • Establish relationships with education personnel to meet education goals.
  • Provide ongoing training and support to foster parents and children.
  • Monitor and review documentation to meet state, licensure and accreditation requirements.
  • Maintain confidentiality as outlined in policy.
  • Assist in Corrective Action Plans (CAP) for foster parents.
