Job summary

Necco is seeking a Foster Care Case Manager to work with children and foster families across office, school, community, and home settings. You will help children reach goals and support foster families for growth and permanency.

The role includes creating treatment plans, coordinating assessments, and ensuring services and legal paperwork are in place. Travel between locations and ongoing training are expected, with a commitment to diverse, inclusive care.

Qualifications

21 years of age.
Current LSW or LPC or Bachelor's degree in a human services-related field (OH, KY, GA).
Bachelor's degree with field experience of 2 years (WV).
Bachelor's degree or Master's degree from an accredited four-year college or university with at least 7 hours in behavioral sciences; one year of casework can substitute for the 7 hours.
Minimum of 2 years experience working with children with emotional or behavioral problems.
Limited Liability Auto Insurance Coverage of 100/300/100.
Valid Driver's License.
Organizational and Written/Oral Communications skills.
Must have the skills to navigate and use software (MS Office, Databases, EHRs, Dashboards).
Successful completion of all required Criminal Background checks and drug screenings per state and company regulations.
Ability to adhere to scheduled and unplanned deadlines.
Willingness to travel and attend training.
Successful completion of all required training pertaining to job.

Responsibilities

Participate in referral matching for children and families, advocating for best home match.
Develop and facilitate Initial Treatment Plans and Comprehensive Treatment Plans for each child.
Facilitate and ensure completion of Assessments (Biopsychosocial, Independent Living, Diagnostic, etc.).
Refer foster child to applicable support services (behavioral health, independent living).
Ensure initial legal documentation is obtained prior to placement or service delivery.
Develop and facilitate each consumer’s Individualized Treatment Plan (ITP).
Assist foster parents and child with implementation and monitoring of the Comprehensive Treatment Plan.
Establish relationships with foster parents and children through training and support.
Meet and/or exceed all state and licensure regulations regarding contact with the foster children on your caseload.
Ensure foster children receive all required medical and therapy services as outlined in the service plan.
Serve as liaison between Necco and placement agencies and community resources.
Participate in service meetings with community representatives, guardians, state workers, clinicians, and schools.
Establish relationships with school and education personnel to support education goals.
Provide ongoing training and support to foster parents and children.
Monitor documentation to meet state/licensure/accreditation requirements.
Maintain confidentiality as outlined in policy.
Assist team members with Corrective Action Plans for foster parents and execute tasks.
Monitor and review medication logs to ensure proper administration.
Ensure discharge planning includes referrals to aftercare services when applicable.
Complete home visits as required by rules.
Deliver services across in-home and community-based settings and travel between locations.
