Foster Care Case Aide - Family Support & Visits

Job summary

SCO Family of Services is seeking a Case Aide to support and supervise family time between parents and children, provide visit coaching, and complete timely documentation. You will collaborate with the case planning team, gather vital documents, and accompany families to appointments across various settings.

The role requires strong communication, a valid NYS driver license, and willingness to travel between office sites.

Qualifications

• Excellent oral and written communication.
• Basic computer literacy including Microsoft Office.
• Bilingual abilities strongly encouraged.
• Valid NYS Driver License required.

Responsibilities

• Support and supervise family time between parents and children, as well as siblings and other resources; provide transportation as needed.
• Provide visit coaching between parents and children to support healthy family engagement.
• Complete ongoing documentation in Connections within 7 business days of the event date.
• Work with the case planning team on Permanency Hearing Reports and court documents related to the case aide’s communication and collaboration with the family.
• Assist case planning staff in gathering vital documents (birth certificates, social security cards, etc.).
• Accompany and support children and parents through appointments including school, medical, and treatment.
• Assist families through Public Assistance and PATH processes and provide referrals as needed.
• Meet with families in their home and/or community as needed.
• Intervene in crisis situations to help stabilize the child and family.
• Attend and testify in court appearances, as needed.
• Attend interdisciplinary team meetings and brief the team on services provided to the child.
• Fulfill Mandated Reporter duties for suspected child abuse or maltreatment.
• Meet with supervisor regularly to discuss safety and permanency issues of caseload.
• Communicate and work with SCO foster care staff and external partners.
• Adhere to SCO’s personnel policies.
• Perform other duties as assigned.
