Qualifications

Responsibilities

• Collect and maintain key documents to adhere to licensing/certification guidelines.
• Communicate with families and specialists; complete visit notes within designated time frames.
• Conduct home visits, including courtesy supervision guardianship reviews and required monitoring visits.
• Travel to Central Phoenix counties, sometimes out of county; visit virtually or in person.
