Olympic Health and Recovery Services is seeking a FHARPS Transitions of Care Coordinator in Lacey, WA. This role bridges behavioral health transitions from inpatient or correctional settings to the community with FHARPS peers and housing supports.

The position focuses on early engagement, collaborative planning, and reducing barriers to improve outcomes for eligible participants, with a Monday–Friday daytime schedule and full-time benefits.

Qualifikationen

• Meets WA state qualifications to be a Mental Health Professional.
• Master’s degree or greater in counseling or social sciences or Bachelor’s with 5 years’ direct behavioral health experience.
• WA DOH license or equivalent (LAAC/CAAC) or related professional license.
• Two years of admin/clinical experience in a social service setting preferred.

Aufgaben

• Screen and track all FHARPS referrals.
• Engage early with individuals to plan discharge or transition needs.
• Collaborate with hospitals and community providers to develop transition plans.
• Conduct outreach to locate and engage FHARPS referrals and participants.
• Educate referred individuals about program requirements and enrollment expectations.
