Qualifications

• Bachelor's degree in social science or related field required.
• 2+ years of relevant experience serving adults with mental illness preferred.
• Ability to navigate electronic medical records and document clearly.

Responsibilities

• Serve as a Primary Case Manager for assigned caseload and coordinate client needs and services; ensure plans and crisis documents are up-to-date.
• Coordinate with internal and external departments on referrals and intake into the program.
• Collaborate with care teams to arrange housing resources, transportation, and access to needed services.
• Maintain client confidentiality and apply trauma-informed care across all interactions.
