The Hampton-Newport News Community Services Board seeks a Forensic Service Coordinator to join our Assertive Community Treatment (ACT) Team, delivering intensive, community-based services to adults with serious mental illness and co-occurring substance use disorders.

You will conduct outreach and assessments, coordinate behavioral health and housing services, provide counseling and crisis intervention, and participate in court-related proceedings as needed to promote stability and recovery.

Formación

• Bachelor's degree in a Human Services field such as Social Work, Psychology, or Counseling.
• One (1) year of experience in case management or service coordination with experience in jails, prisons, and criminal justice agencies.
• Must be registered with the Board of Counseling to practice as a QMHP-A or register as a QMHP-A trainee within 30 days of employment.
• Master's Degree preferred.

Responsabilidades

• Conduct outreach, interviews, mental status and psychosocial assessments, and develop individualized treatment plans with the ACT Team.
• Coordinate behavioral health, substance use, medical, housing, financial, social, and community-based services; make referrals and advocate for resources.
• Provide supportive counseling, crisis intervention, daily living assistance, medication monitoring, and substance use screenings.
• Provide specialized service coordination for individuals with forensic involvement, collaborating with legal and community partners, and participating in discharge planning as required.
• Monitor client progress and maintain timely service coordination documentation.
