Hampton-Newport News Community Services Board is seeking a Forensic Service Coordinator in Hampton, VA. The role is part of the Assertive Community Treatment (ACT) Team, delivering intensive, coordinated services to adults with serious mental illness and co-occurring disorders.

You will conduct outreach, assessments, and develop individualized plans, coordinating behavioral health, housing, and supports while liaising with courts and community partners to promote stability and recovery.

Competenze

• Bachelor's degree in a Human Services field such as Social Work, Psychology, or Counseling.
• One year of experience in case management or service coordination, including work with jails, prisons, and criminal justice agencies.
• Must be registered with the Board of Counseling to practice as a QMHP-A or register as a QMHP-A trainee within 30 days of employment.
• Master's Degree preferred.

Mansioni

• Conduct outreach, interviews, mental status and psychosocial assessments; develop, monitor, and document individualized treatment plans with the ACT Team.
• Coordinate behavioral health, substance use, medical, housing, financial, social, and community-based services; make referrals and advocate for resources.
• Provide supportive counseling, crisis intervention, daily living assistance, medication monitoring, and substance use screenings to promote treatment adherence.
• Provide specialized service coordination for individuals with forensic involvement, collaborating with legal and community partners, participating in discharge planning, and attending or testifying at court hearings as required.
• Monitor client progress through community and residential visits; maintain timely service coordination documentation.
