Job summary

City of Virginia Beach seeks a BH/DS Clinician III for the ACT program to work with forensic clients reintegrating into the community. The role focuses on coordination with jails, hospitals and social services, plus comprehensive assessments and treatment planning.

Qualifications

• Requires QMHP or higher licensed clinician (LCSW/LPC/LCP/LMFT)
• Four-year degree in a human services field with required experience
• DMV license valid

Responsibilities

• Coordinate services with jails, prisons and hospitals for forensic clients reintegrating into the community.
• Provide comprehensive assessments for forensic individuals with chronic mental illness and co-occurring disorders.
• Develop individualized treatment plans and coordinate service delivery with the treatment plan.
• Provide linkage, monitoring and coordination of medical care, therapy, housing, and vocational services.
• Assist clients with appointments and meetings with Social Security, landlords and community services.
• Monitor progress and complete forensic reports for state hospitals, jails, and probation/parole.
• Serve as primary care coordinator (SAI) for ACT caseload of 10–15 clients.
• Provide counseling, outreach visits, crisis intervention and advocacy.
• Deliver individual, group, family counseling and independent living training.
• Assist with medical services, scheduling and transport to appointments.
