Mental Health Cooperative is seeking a FACT Care Manager for Forensic Services in Nashville, TN. The role focuses on comprehensive care management for youth and adults with serious mental illness, coordinating with an interdisciplinary team to support community-based goals.

Responsibilities include engaging with consumers, conducting visits across settings, addressing barriers to engagement, and ensuring quality documentation while meeting productivity standards.

Qualifications

• Bachelor’s Degree in a health-related field required.
• Candidates with a criminal justice degree must have 15 college level hours in behavioral health.

Responsibilities

• Engage with consumers monthly to assess needs and coordinate care.
• Complete community visits to evaluate needs and connect with individuals.
• Address mental and physical health, social determinants of health, gaps in care, and ED/IP utilization.
• Document thoroughly following agency policies and procedures.
• Collaborate with consumers’ support system and community partners.
• Participate in daily team meetings and monthly supervision to review caseloads.
