Job summary

Pathways Vermont is seeking a Forensic Assertive Community Treatment (FACT) Program Manager to lead the FACT team in Burlington, VT. The role oversees operations, staff supervision, and collaboration with DOC/DMH, Housing First, and funders to deliver high‑quality, Medicaid‑compliant services.

The Program Manager ensures program fidelity to the FACT model, drives measurable outcomes, and manages budgets, grants, and policy development while maintaining community partnerships and service

• Valid driver’s license, a reliable vehicle for work purposes.
• Lived experience of incarceration is a strong asset.
• Personal experience of homelessness or mental health challenges is an asset.

Responsibilities

• Oversee day-to-day program operations of the FACT team and relationships with clients.
• Hold primary responsibility for the quality, effectiveness, consistency, and sustainability of the FACT program.
• Develop, implement, and regularly evaluate annual program goals and performance measures.
• Manage the FACT grant and prepare grant reports for quarterly meetings.
• Ensure compliance with grant, contract, Medicaid and confidentiality requirements.
• Manage the FACT program budget with finance and leadership; monitor expenditures.
• Provide oversight of incident reporting and program risk management.
• Lead development and review of FACT-specific policies and staff onboarding.
• Supervise FACT team members, conduct weekly supervision, and provide coaching.
• Support hiring decisions, staffing structures, and coverage plans.
• Engage with DOC, DMH, Housing First and funders to advance program goals.
• Provide direct services to participants as needed and ensure continuity of services.
