Mental Health Cooperative in Nashville, TN is seeking a FACT Care Manager for Forensic Services. The role focuses on comprehensive care management for youth with SED and adults with SPMI, coordinating care and collaborating with an interdisciplinary team to improve community outcomes.

The position requires a health-related bachelor’s degree, valid TN driver's license, and strong organizational skills. The team emphasizes recovery, collaboration, and integrity in a supportive work environment.

Qualifications

• Bachelor’s degree in a health-related field required.
• Candidates with criminal justice degree must have 15 college-level credits in behavioral health.
• Valid Tennessee Driver’s License and acceptable MVR and background check.
• Personal automobile insurance required.

Responsibilities

• Provide comprehensive care management services to children, youth and adults with SED/SPMI.
• Engage with consumers monthly to assess needs, develop goals, and coordinate care.
• Complete community visits (home, school, court, etc.) to evaluate needs and connect with individuals.
• Address mental/physical health, social determinants, care gaps, and ED/IP utilization.
• Maintain timely, accurate documentation per agency policies; meet productivity standards.
• Collaborate with team members and community partners to develop actionable plans.
