Job summary

Project 180 is seeking a Case Manager (Interim) in Los Angeles, CA to deliver intensive, wraparound field-based case management for individuals diverted to community-based competency restoration. Under supervision, you will engage clients, facilitate access to resources, and collaborate with a multidisciplinary team to support recovery goals.

End date Oct 31, 2026 with possible permanency. Responsibilities include motivational interviewing, linkage to services, documentation, and coordination

Qualifications

• BA/BS in any field required.
• 1 year of case management experience or 2 years if no BA.
• Valid CA driver’s license, proof of insurance, and ability to transport clients.
• Must not be on probation or parole; background check required.

Responsibilities

• Actively engage clients with strong rapport and motivational interviewing.
• Provide wraparound, field-based case management and coordinate with a multidisciplinary team.
• Develop linkages to community resources and monitor ongoing service participation.
• Document case notes in the electronic health record per guidelines.
• Engage with collateral contacts (e.g., family, probation, court) and provide client advocacy.
