LifeStream Behavioral Center, Inc. is seeking a Forensic Case Manager in Leesburg, FL.

You will join a multidisciplinary team to support individuals returning from or diverted from forensic hospital placement and coordinate community-based care. The role emphasizes collaboration with families, courts, DCF, and community agencies, ensuring timely documentation and discharge planning while maintaining compliance with court requirements.

Qualifications

• Bachelor's Degree in Human Services or related field required.
• Certification as targeted case manager within six (6) months of hire required.
• Minimum 1 year experience in behavioral healthcare or 1 year experience in criminal justice required.

Responsibilities

• Collaborate with individuals, families, courts, DCF, staff and community agencies to support living independently in the community.
• Remain involved in the individual's care and discharge planning if forensic hospitalization occurs.
• Monitor progress and link individuals to appropriate services.
• Complete required documentation accurately and timely.
• Develop person-centered plans in collaboration with the multidisciplinary team, aligned with court requirements.
• Link individuals to services beyond the program.
