Qualifications

• New York State licensure as LMSW, LCSW, LMHC, or other applicable credentials.
• Three+ years experience serving adults with serious mental illness or related needs.
• At least one year experience with individuals involved in the criminal justice system.
• Experience in ACT/FACT or integrated behavioral health/criminal-justice settings preferred.
• Strong commitment to rights, autonomy, and recovery for individuals with mental illness.
• Bilingual/multilingual abilities are a plus.
• Valid NY driver’s license; access to reliable vehicle.

Responsibilities

• Conduct comprehensive assessments and develop individualized treatment plans.
• Provide clinical and support services in office and community settings.
• Coordinate clinical services, crisis intervention, medication support, and case management.
• Assess needs and develop strategies to promote stability and reduce recidivism.
• Respond to crises and participate in on-call rotations.
• Engage with courts, probation, parole, and treatment teams; refer to community resources.
• Build relationships with community stakeholders and partners.
• Provide transportation when clinically necessary.
