Job summary

Mohawk Opportunities Inc. seeks a Clinician – Criminal Justice Specialist to join the FACT team serving Albany and Schenectady counties.

You will assess, plan, and coordinate integrated treatment for adults with complex needs, including serious mental illness and co-occurring disorders, in office and community settings. You will coordinate with probation, parole, treatment courts, and other justice-system partners, providing crisis intervention and linkage to community resources as needed.

Qualifications

• Licensed clinical social work or equivalent in New York State.
• Minimum three years of experience with adults with serious mental illness.
• Experience with individuals involved in the criminal justice system preferred.

Responsibilities

• Conduct comprehensive facially-structured assessments and create individualized treatment plans.
• Provide clinical and support services in office and community settings including homes, shelters, and courts.
• Coordinate services across justice-system partners and ensure safety and crisis intervention as needed.
• Lead on criminogenic risk/needs assessment and integrate findings into treatment planning.
• Collaborate with stakeholders to promote community stability and reduce recidivism.
