Job summary

Pinnacle Home Care in Florida is hiring a PRN Speech Language Pathologist (SLP) to evaluate and treat patients in their homes. You will assess communication and swallowing, develop individualized plans, and collaborate with physicians and an interdisciplinary team.

The role requires a Florida license, a Master’s in Speech-Language Pathology, at least two years of clinical experience, and a current BLS certification.

Qualifications

• Graduate of an accredited Speech Therapy program.
• Licensed Speech Therapist in Florida.
• Minimum of two years of clinical experience as a Speech Therapist.
• BLS certification current.
• Valid driver's license and insured, reliable vehicle.
• Ability to stand, stoop, and bend frequently; strong stamina.

Responsibilities

• Evaluate clients’ speech, language, and hearing abilities and recommend communication strategies.
• Conduct initial and ongoing assessments, including OASIS evaluations.
• Develop and revise treatment plans with physicians as needed.
• Collaborate with interdisciplinary team on care plans and provide training.
• Educate clients and families on therapy techniques and challenges.
• Document all evaluation data, treatments, and progress promptly.
• Supervise Home Health Aides every 14 days when Speech Therapy is the sole discipline.
• Communicate with physicians about orders and changes in client condition.
