Pinnacle Home Care, Florida’s largest Medicare-certified home health provider, is hiring a PRN Speech Language Pathologist (SLP) to join our award-winning team delivering care at patients’ homes across multiple counties. The role involves evaluations, treatment planning, and interdisciplinary collaboration.

We seek a licensed Florida SLP with at least two years of clinical experience, BLS, and a valid driver’s license.

Qualifications

• Graduate of an accredited Speech Therapy program.
• Licensed Speech Therapist in Florida.
• Minimum of two years of clinical experience as a Speech Therapist.
• Current BLS Certification.
• Valid driver’s license and insured, reliable vehicle.

Responsibilities

• Evaluate clients’ speech, language, and hearing abilities and recommend strategies to enhance communication.
• Conduct initial and ongoing comprehensive assessments, including OASIS evaluations.
• Develop and revise treatment plans in consultation with physicians.
• Collaborate with interdisciplinary team members on care plans and provide speech pathology training as needed.
• Educate clients and families on speech therapy techniques and challenges.
• Document all evaluation data, treatments, and client progress promptly.
• Supervise Home Health Aides every 14 days when Speech Therapy is the sole discipline involved.
• Communicate with physicians to ensure orders are appropriate and adjust as needed.
• Participate in periodic revision of care plans and update physicians on client status.
