Job summary

Pinnacle Home Care is seeking a PRN Speech Language Pathologist (SLP) to join our Florida team. You will evaluate speech, language, and hearing, develop treatment plans, and collaborate with physicians to ensure optimal patient outcomes in the home setting.

Qualifications

• Graduate of an accredited Speech Therapy program.
• Licensed Speech Therapist in Florida.
• Minimum of two years of clinical experience as a Speech Therapist.
• Current BLS Certification.
• Valid driver’s license and insured, reliable vehicle.
• Strong emotional resilience and physical stamina.
• Excellent judgment, communication, and observational skills.
• Self-motivated with the ability to work independently.

Responsibilities

• Evaluate clients’ speech, language, and hearing abilities and recommend strategies.
• Conduct initial and ongoing comprehensive assessments including OASIS evaluations.
• Develop and revise treatment plans in consultation with physicians.
• Collaborate with interdisciplinary team and provide speech pathology training as needed.
• Educate clients and families on speech therapy techniques.
• Document evaluation data, treatments, and progress promptly.
• Supervise Home Health Aides every 14 days when Speech Therapy is the sole discipline involved.
• Communicate regularly with physicians and update orders as needed.
• Participate in periodic care plan revisions and obtain additional orders as required.
