Job summary

Pinnacle Home Care is seeking a PRN Speech Language Pathologist (SLP) to join our award-winning team in Florida. You will evaluate and treat patients at home, collaborate with an interdisciplinary team, and educate families on therapy techniques while maintaining thorough documentation.

The ideal candidate has a Florida license, a graduate degree in Speech Therapy, at least two years of clinical experience, and a current BLS certification.

Qualifications

• Graduate of an accredited Speech Therapy program.
• Licensed Speech Therapist in Florida.
• Minimum of two years of clinical experience as a Speech Therapist.
• Current BLS Certification.
• Valid driver’s license and insured, reliable vehicle.

Responsibilities

• Evaluate clients’ speech, language, and hearing abilities and recommend strategies to improve communication and hearing.
• Conduct initial and ongoing comprehensive assessments, including OASIS evaluations.
• Develop and revise treatment plans with physicians as necessary.
• Collaborate with interdisciplinary team and provide speech pathology training to team members as needed.
• Educate clients and families on speech therapy techniques and related challenges.
• Document all evaluation data, treatments, and client progress promptly.
• Supervise Home Health Aides every 14 days when Speech Therapy is the sole discipline involved.
• Communicate regularly with physicians to ensure orders are appropriate and address changes in client condition.
• Participate in periodic revision of care plans, updating the physician on client status and obtaining additional orders as required.
