Qualifications

• Active Minnesota Registered Nurse license.
• Strong clinical judgment and adaptability.
• Comfortable working independently in a field-based/home health environment.

Responsibilities

• Float between home health territories throughout the Twin Cities based on patient needs.
• Complete skilled nursing visits and OASIS assessments.
• Develop and update individualized care plans with the interdisciplinary team.
• Coordinate care with physicians and other clinicians.
• Provide compassionate, one-on-one home health care and maintain timely documentation.
