Job summary

Home Health Care, Inc. in the Greater Twin Cities Metro Area is seeking a Registered Nurse (RN) Case Manager for Floating Territories.

You will work across multiple home health territories, delivering autonomous, patient-focused care while building trusted relationships with patients and teams. Responsibilities include completing OASIS assessments, developing individualized plans of care, and conducting one-on-one nurse visits with patient education.

Qualifications

• Active Minnesota RN license.
• Strong clinical assessment and critical thinking.
• Willingness to travel throughout the Twin Cities metro.
• Home health experience preferred but not required.

Responsibilities

• Provide skilled nursing support across multiple home health territories throughout the Twin Cities.
• Complete OASIS assessments and individualized plans of care.
• Deliver one-on-one skilled nursing visits and patient education.
• Collaborate with physicians and interdisciplinary team members.
• Support local teams during staffing needs and coverage.
• Maintain accurate and timely clinical documentation.

Location: Greater Twin Cities Metro Area (21-county service area)

Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM | On-Call Rotation

Compensation: $95,000 – $130,000 annually plus incentive visit opportunities and $6,000 in first-year bonuses
