Job summary

Monarch Healthcare seeks dedicated Floaters to support individuals with mental health needs, developmental disabilities, or substance use disorders. You will help people pursue personal goals and build community connections while maintaining safety and documentation.

Qualifications

• High School Diploma is required.
• Valid Driver's License (USA) is required.
• Experience with individuals with mental health diagnoses, developmental disabilities, or substance use disorders.
• Flexibility to work various shifts including nights, weekends, and holidays.

Responsibilities

• continually assess each person’s hopes, dreams, and desires and provide supports to achieve them.
• Help individuals develop relationships in their community and with natural supports.
• Assist individuals in participating fully in their community according to their interests.
• Protect and promote the rights of individuals receiving services; support advocacy.
• Provide emotional, physical, and medical support as needed to each person.
• Educate individuals on procedures, policies, and rights; ensure safety and compliance.
• Foster person-centered, positive relationships encouraging self-determination.
• Assist with assessment and planning input and document progress and outcomes.
• Complete daily progress notes and maintain records such as attendance and logs.
• Assist with home chores and daily living tasks as needed.
• Ensure safety in community settings and during activities; conduct drills and safety checks.
• Supervise and mentor staff in duties; cover shifts as needed.
