Qualifications

• Current RN license
• Minimum 2 years of recent acute care Medical-Surgical nursing experience
• Strong clinical judgment, adaptability, and communication skills
• Ability to work independently in a variety of clinical settings

Responsibilities

• Promotes patient and family centered care and educates on post-treatment home care needs
• Participates in patient safety initiatives and professional development
• Develops and documents individualized plans of care and communicates across care continuum
• Assesses patient needs and barriers, including cultural and emotional considerations, to guide care
