Job summary

Community Hospice is seeking a Registered Nurse Case Manager (Float) to plan and oversee nursing care for a caseload in home and long-term care settings. You will collaborate with the interdisciplinary team to achieve patient- and family-directed goals using a holistic approach, provide education, coordinate care, and ensure timely documentation.

This role requires Florida RN licensure, CPR, and flexibility to work across care settings, including extended and irregular hours, with strong

Qualifications

Responsibilities

• Assess and identify patient and family goals and needs.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans.
• Participate in team meetings and ethical discussions.
• Educate patients and caregivers about disease progression, medications, nutrition, and hospice philosophy.
• Coordinate timely and effective responses to changing or urgent patient needs.
• Maintain accurate, timely, and compliant documentation.
• Support and educate team members, patients, families, and the broader community.
• Participate in internal quality improvement initiatives.
• Facilitate interdisciplinary group (IDG) meetings in the absence of the manager.
• Perform other duties as assigned.
