Job summary

Community Hospice is seeking an RN Case Manager (Float) to plan and manage nursing care for an assigned patient caseload across home and long-term care settings. You will collaborate with the interdisciplinary team to achieve patient and family goals using a holistic, patient‑centered approach and provide education and coordination of care.

Optional flexibility across care sites is required. The role requires current Florida RN license, CPR cert, and an ADN/BSN.

Qualifications

• Strong communication skills (verbal, written, non‑verbal).
• Understanding of hospice/palliative nursing principles.
• Cultural competence and ethical decision‑making in care.

Responsibilities

• Assess patient/family goals and needs.
• Develop and implement individualized care plans with the interdisciplinary team.
• Participate in team meetings and ethical discussions.
• Educate patients and caregivers on disease progression, meds, nutrition, hospice philosophy.
• Coordinate timely responses to changing patient needs.
• Maintain accurate, timely, compliant documentation.
• Support and educate team, patients, families, and community.
• Participate in quality improvement initiatives.
• Facilitate IDG meetings in manager's absence.
• Perform other duties as assigned.
