Job summary

Concentra in Chicago is seeking a Physical Therapist to provide rehabilitative services for injured and disabled patients within our centers. You will work with the Injury Process Management and rehabilitation philosophy in occupational medicine, collaborating with medical staff to ensure comprehensive care.

You will deliver direct patient care, perform evaluations of musculoskeletal and neurological issues, and maintain thorough documentation while upholding HIPAA and company policies.

Qualifications

• Bachelor's Degree from an accredited Physical Therapy program.
• Master's Degree in Physical Therapy is preferred.
• Ortho-outpatient experience is preferred.

Responsibilities

• Ensure delivery of high quality patient care services
• Perform direct patient care, including evaluation of muscle, skeletal, neurological and other physical problems
• Support and encourage multidisciplinary coordination of treatment
• Ensure proper documentation of patient care
• Maintain professional standards through QA policies and applicable regulations
• Communicate effectively with staff, physicians, and patients to ensure coordinated care
• Plan, design and interpret physical therapy treatment programs
• Educate patients on proper care and exercise programs
