Qualifications

• Licensed pharmacist in WI or eligible for reciprocity.
• Immunization and non‑vaccine injectable certification required.
• Credentialing and privileging based on area of practice.

Responsibilities

• Oversees pharmaceutical care and evaluates biomedical literature for therapy regimens.
• Provides patient and caregiver counseling on medications, adherence, and safety.
• Designs, monitors, and adjusts pharmacotherapeutic regimens per guidelines and policies.
• Supports formulary management, medication safety, and quality improvement activities.
• Precepts students and mentors pharmacy teammates and trainees.
