Job summary

UnitedHealth Group is seeking a Registered Nurse in Home Health to deliver nursing care in patients' homes as prescribed by physicians, coordinating with the care team to ensure continuity of care and optimal outcomes.

The role requires an unrestricted RN license, at least 1 year of home health experience, CPR certification, and a valid driver's license with access to a reliable vehicle. You will work within a supportive team and participate in on-call rotations as needed.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• 1+ years of Home Health experience.
• Current CPR Certification or ability to complete within 90 days of hire.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Ability to function in any home situation regardless of client demographics.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical nurses, provides instruction, and assigns tasks according to regulations.
• Provides required supervisory visits.
