UnitedHealth Group is seeking a Registered Nurse in Home Health to provide and direct nursing care to patients in their homes, coordinating with physicians and the care team to ensure continuity of care. The role involves developing and revising the plan of care, supervising home health staff, and ensuring high-quality clinical services in line with regulations.

Required qualifications include an unrestricted RN license and a valid driver’s license with access to reliable transportation.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred CPR Certification and 1+ years of Home Health experience.

Responsabilités

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed nurses, provides instruction, and assigns tasks according to State and federal regulations.
• Provides required supervisory visits and ensures compliance with policies and regulations.
• Completes assessments (OASIS), medication reconciliation, and documents visits per policy and payer requirements.
