Job summary

UnitedHealth Group is seeking a Registered Nurse in Home Health to deliver and coordinate nursing care in patients' homes, in line with physician orders and agency policies. You will work with a multidisciplinary team to ensure continuity of care, supervise aides and LVNs, and complete OASIS assessments and accurate documentation.

Required: current RN license, valid driver’s license, and reliable transportation. CPR certification preferred or obtainable within 90 days.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification preferred or able to complete within 90 days of hire.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with the physician and other health care professionals.
• Supervises home health aides and licensed nurses; provides instruction and assigns tasks per state and federal regulations.
• Provides supervisory visits as required and conducts comprehensive nursing assessments (OASIS).
• Documents visits and ensures coding and payer requirements are met; maintains infection control standards.
